Current Year Contribution Arrears Notice
When a plan member falls into current year arrears, the employer must notify SHEPP by completing a Current Year Contribution Arrears Notice. Then, through current year payroll processing, calculate, collect and remit the principal arrears contribution prior to the end of the current payroll year.

 

Designation of Beneficiary
Provide a Designation of Beneficiary form to new Plan members so that they may designate their beneficiaries, and to existing Plan members who wish to change their beneficiaries. Active Plan members can also update their beneficiaries on SHEPPWeb.

 

Enrolment
Enrol eligible employees in the Plan with the Enrolment form.

 

Employer's Statement of Disability
This is one of three forms needed for members to obtain SHEPP-approved disability status (along with Member Application for Pension Accrual While Disabled and Physician's Statement of Disability), all of which are sent to a third party adjudicator for approval. If approved, the member's pension will continue to accrue. This form is to be filled out by the employer.

 

Leave of Absence Purchase Member Notification
When a Plan member's request for unpaid leave of absence has been approved by the employer, both employer and member must complete a Leave of Absence Purchase Member Notification and submit it to SHEPP prior to the date the leave begins.

 

Member Change of Information
To change a member's biographic and/or spousal information a Member Change of Information form must be completed and submitted.

 

Member's Application for Pension Accrual While Disabled
This is one of three forms needed for members to obtain SHEPP-approved disability status (along with Physician's Statement of Disability and Employer's Statement of Disability), all of which are sent to a third party adjudicator for approval. If approved, the member's pension will continue to accrue. This form is to be filled out by the member.

 

Monthly Contribution Report
Member and employer contributions must be remitted each month, along with a Monthly Contribution Report. The deadline for remitting contributions is the 15th day of the following month.

 

Notice of Termination of Plan Membership
When a member's employment ends, employers must complete and submit a Notice of Termination of Plan Membership with final earnings, contributions and service information for the member.

 

Payroll Contribution Adjustment Notice
Adjustments that are not arrears must be reported using the Payroll Contribution Adjustment Notice so that correct earnings and hours are appropriated each month.

 

Physician's Statement of Disability
This is one of three forms needed for members to obtain SHEPP-approved disability status (along with Member Application for Pension Accrual While Disabled and Employer's Statement of Disability), all of which are sent to a third party adjudicator for approval. If approved, the member's pension will continue to accrue. This form is to be filled out by the member and their physician.

 

Prior Year(s) Contribution Arrears Notice
When a Plan member falls into prior year arrears, the employer must notify SHEPP by completing a Prior Year(s) Contribution Arrears Notice. SHEPP will calculate the contribution arrears due to the plan. The arrears must be collected and remitted outside current year payroll processing as there are special tax reporting and accounting procedures for prior year arrears.

 

Request for Pension Estimate
Employers may provide a Request for Pension Estimate form to members who would like to request a SHEPP pension projection be prepared and sent by mail. Active Plan members may perform unlimited pension projections themselves using SHEPPWeb.

 

SHEPPWeb Employer Access Agreement
SHEPPWeb Employer Registration Package
To be provided with access to SHEPPWeb for Employers, an organisation must register its authorised officer(s). They can appoint up to 12 administrative user who must also abide by the terms and conditions of the SHEPPWeb Employer Access Agreement.

Transfer of Benefits
Complete and submit a Transfer of Benefits form to transfer a member's contribution and credited service to another SHEPP participating employer, pursuant to the terms for SAHO/CUPE or SAHO/SUN collective agreements.
